A user's Terminal Server session that is not disconnected can cause login and password issues.
When a user changes his password for any reason -- i.e., forgotten password, expired password, etc. -- the user will sometimes appear to be locked at random intervals in the Active Directory. If you have some/any Terminal Servers in your network, check to see if they have any forgotten disconnected sessions left. These sessions may be the cause of the lockout.
You can set a limit on the maximum disconnected session time in the RDP configuration to avoid these problems in the future.
